Truentity Health’s mission is to help reduce medication-related errors and optimize medication therapies for providers, pharmacies and hospital systems. Before and after a patient’s hospital visit, a licensed pharmacist conducts a comprehensive medication reconciliation (medrec) process to compile a complete record of their prescription medications, over-the-counter (OTC) drugs, supplements, drug allergies and past adverse reactions. This record is then securely transmitted over the Truentity HealthX network to the hospital system’s EHR, giving the provider a holistic view of the patient’s medication history to help reduce errors and optimize therapies.
