Visit Nationwide Connected Care at the HIMSS Interoperability Showcase!
This dedicated vignette will demonstrate how a patient is able to better coordinate his care across multiple hospitals and physicians thousands of miles apart. Attendees will see CommonWell services embedded in participating members EHRs, PHRs and software, and watch as his care is seamlessly handed off across the disparate facilities, vendors and technologies.

Meet Oscar
Oscar, 36, is a traveling nurse. At home in upstate New York, he’s diagnosed with MS. Months later, while on assignment in Louisiana, Oscar acquires a respiratory infection caused partially by his MS immunocompromising medication and is diagnosed with Pneumonia. Follow his health journey through acute inpatient, standalone ED, PCPs, specialty and urgent care settings, as well as his health management via his PHR in his consumer app of choice, to manage his chronic and acute conditions. Watch as his providers leverage event notifications, smart record locator services and coordinate care to help avoid readmission. Regardless of where his care occurs, Oscar and his providers have access to his health records nationwide via CommonWell Health Alliance, resulting in positive health outcomes.
